Semen Analysis

Please call the Assisted Reproductive Technology (ART) Unit at (852) 3505 1456 to book an appointment for semen analysis test.

You are advised to abstain from sexual intercourse or ejaculation for at least 2 days but not more than 7 days before semen analysis test.

Please call (852) 3505 1456 to book an appointment.

Step 02

Registration

Please bring the following documents to your appointment:

Hong Kong l.D. card and

‘Referral for Semen Test at Andrology Laboratory, IVFFHK’ form (with referral).

Download and complete the ‘Walk- in Request Form for Andrology Tests at Andrology Laboratory’ (without referral).

Step 03

Semen Collection

You need to produce your semen sample in the Semen Collection Room of the ART Unit.

Please read the ‘Instructions for Semen Collection’ before you produce the sample.

Step 04

Settle Payment

After semen collection, please settle the payment at the reception.

Then submit the sample to the Andrology Laboratory.

Step 05

Report Collection

You can collect your report at the reception of ART Unit from 7 working days to 3 months after the test. Report can be collected by the following means:

In person – please bring the completed “Request Form for Collection of CUHK Medical Report (including Semen Analysis Report)” and present your H.K. l.D. Card for verification

By an authorized representative – the representative should present his/her H.K. I.D. card, a copy of your H.K. I.D. card and the completed “Request Form for Collection of CUHK Medical Report (including Semen Analysis Report)” and “Request Form for Collection of Medical Report by other person”
